Job summary

Mercy Health in Lima, OH, seeks a Licensed Independent Social Worker (LISW) to provide advanced clinical social work services to patients, families and groups across the continuum of care. You will assess, diagnose, treat, and coordinate care with an interdisciplinary team while ensuring compliance with licensure and policy.

You will conduct psychosocial assessments, deliver individual, group and family therapy, manage crisis intervention and care planning, and document encounters per standards.

Qualifications

• MSW required (Master’s degree in Social Work).
• Licensed Independent Social Worker (LISW) or Licensed as a Clinical Social Worker (LCSW) per state rules.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
• Provide evidence-based individual, group, and family therapy.
• Deliver crisis intervention, risk assessments, and safety planning.
• Diagnose and treat mental health disorders within scope of practice.
• Coordinate referrals to community resources and higher levels of care.
• Document all patient encounters per professional, legal, and regulatory standards.
