Job summary

Mercy Health – Perrysburg Medical Center is actively seeking a Licensed Independent Social Worker to provide advanced clinical social work services across the continuum of care, including assessment, diagnosis, treatment planning, psychotherapy, crisis intervention, and care coordination.

The LISW will work independently and with an interdisciplinary team to support emotional and psychosocial needs while ensuring compliance with licensure laws, ethical standards, and organizational policies.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW) as required in state of practice.

Responsibilities

• Conduct psychosocial assessments and develop individualized treatment plans.
• Provide individual, group, and family therapy.
• Deliver crisis intervention, risk assessments, and safety planning.
• Diagnose and treat mental health disorders within scope of practice.
• Coordinate referrals to community resources and higher levels of care.
• Document all patient encounters in accordance with standards.
