Job summary

Northbay Integrative Primary Care is seeking a Lifestyle Medicine Health Coach/Nutritionist to empower patients to adopt sustainable lifestyle changes. You will coach on nutrition, activity, sleep, stress, and weight management, collaborating with physicians and an interdisciplinary team.

Qualifications

Bachelor's degree in nutrition or related field required; Master's preferred.
Registered Dietitian certification required; lifestyle medicine certifications preferred.
Minimum of two years health coaching, wellness, nutrition counseling, lifestyle medicine, or preventive health experience.
Experience utilizing motivational interviewing and behavior change methodologies.
Excellent interpersonal, communication, and patient engagement skills; EMR documentation experience.
Familiarity with culinary medicine programming preferred.

Responsibilities

Provide individualized lifestyle medicine coaching focused on sustainable behavior change, nutrition, activity, sleep, stress management, weight management, and chronic disease prevention.
Partner with patients to set meaningful health goals and develop personalized action plans reflecting values and readiness.
Utilize evidence-based coaching techniques including motivational interviewing, positive psychology, and behavior change science.
Educate patients using practical, food-first nutrition principles emphasizing whole foods, culinary medicine, and sustainable eating patterns.
Identify barriers to behavior change and collaborate to develop realistic strategies for adherence and self-management.
Document patient encounters, progress toward goals, coaching interventions, and care plans accurately in the EMR.
Collaborate with physicians and the interdisciplinary care team to coordinate individualized patient care plans.
Facilitate individual visits, group medical visits, workshops, and educational classes; support culinary medicine programming.
