NorthBay Health is seeking an Integrative Primary Care RN to partner with the Program MDs and interdisciplinary team in a concierge-like care model focused on lifestyle medicine.

Responsibilities include complex care management, coaching, and coordinating with specialists to empower patients toward meaningful, sustainable health changes. Requires BSN, California RN license, and at least two years of RN experience.

Qualifikationen

• Graduated from an accredited nursing school and a California RN license.
• BSN is required for this role.
• Completion of American College of Lifestyle Medicine course within 30 days is required.
• Knowledge of CMS, TJC and EMTALA standards relevant to ambulatory care.
• Proficiency in lifeStyle Medicine concepts and patient coaching.

Aufgaben

• Collaborate as part of a 1:1 MD/RN care team within the Integrative Primary Care Program.
• Manage complex outpatient care for patients with chronic conditions.
• Perform independent RN duties in an outpatient setting including patient assessment and care coordination.
• Take vitals and maintain exam rooms; prepare and process lab specimens.
• Counsel patients and coordinate referrals and follow up.
• Schedule testing and fulfill EHR documentation requirements.
• Engage in patient communication and education with the care team.
• Develop educational materials reflecting lifestyle medicine principles.
• Promote high-touch, high-service patient experiences.
