Responsibilities
Lifestyle Medicine program development & coordination
Help support the growth and development of the practice’s lifestyle medicine services including patient education, one‑on‑one support, group visits, and wellness programs.
Utilize a Bible-based lifestyle medicine framework to support physical, emotional, and spiritual wellness.
Patient Education & Individual Support
Coordinate the Chronic Care Management program & care plans for eligible members offering one‑on‑one support (Medicare CCM).
Group Support
Coordinate group lifestyle medicine visits with Physicians / Providers facilitating the communication with patients and helping to create a positive experience.
Other clinical and administrative tasks based on experience may include:
Registered Dietitian / Nutrition Counseling, Medical Assisting, Lifestyle Medicine Health Coaching, Community / Church Wellness Event Support.
