Qualifications

• Bachelor’s degree in social work, education, psychology, or related field, or equivalent experience.
• At least one year of relevant training experience in personal assistance agency or related health programs.
• Excellent oral and written communication and presentation skills.
• Professional demeanor and appearance.
• Proficient skills to promote client relations with a positive attitude.
• Strong interpersonal, communication, and time management skills; ability to work independently and collaboratively.
• Proficiency with office software and electronic record-keeping systems.

Responsibilities

• Program Design & Delivery: Develop and deliver individualized life skills training plans aligned with members' goals.
• Direct Support & Coaching: Provide regular life skills support during transitions or life changes.
• Community Integration: Facilitate access to community resources, counseling, and employment services.
• Collaboration: Work with care teams, case managers, healthcare professionals, and other providers.
• Resource Navigation: Guide members to community services and communicate with case workers.
• Documentation: Maintain confidential case notes, progress reports, and program evaluations.
• Conflict Resolution & Safety: Assist with conflict resolution and ensure member safety.
• Program Evaluation: Monitor progress and adjust training plans as needed.
