Rehab Without Walls Neuro Rehabilitation, part of BrightSpring Health Services, is seeking a Licensed Clinical Social Worker (LCSW) for our Home & Community Program in Seattle, WA. This is a PRN W2 position with flexible scheduling and in-person 1:1 care.

Provide psychosocial counseling in home and community settings, help clients return to work, coordinate with an interdisciplinary team, and support recovery after brain or spinal injuries. CPR is required and lifting duties may apply.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Current state Licensure as LCSW required.
• Minimum 1 year rehab setting experience with brain/spinal injuries.
• CPR certification required.
• Physical ability to lift up to 50 pounds and assist moving patients.
• Moving, lifting, or transferring patients up to 100 pounds with safety procedures.
• Standing/walking for extended periods.

Responsibilities

• Provide counseling and psychosocial support in home and community settings.
• Help clients navigate returning to work, driving, and increased community participation.
• Support emotional, cognitive, and behavioral adjustment following injury or illness.
• Collaborate with an interdisciplinary team including OTs, PTs, and SLPs.

Rehab Without Walls® is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community Program in Seattle. This is a W2 PRN position (not 1099), offering flexibility with the backing and support of an established interdisciplinary care team.

Our innovative model brings outpatient‑level services directly to clients, supporting recovery and reintegration in the environments where life actually happens.

Responsibilities

What you will do: (responsibilities listed are included but not limited to)

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems
• Instructs/counsels patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient/ and/or family to utilize appropriate community resources to achieve measurable objectives

Qualifications

What you’ll need:

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• CPR certification required
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods
• Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
