Overview

Licensed Clinical Social Worker (LCSW) – PRN (W2).

Rehab Without Walls Home & Community Program, Bellevue, WA.

Rehab Without Walls is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community Program in Seattle. This is a W2 PRN position (not 1099), offering flexibility with the backing and support of an established interdisciplinary care team.

Our innovative model brings outpatient level services directly to clients, supporting recovery and reintegration in the environments where life actually happens.

What You’ll Do:

• Provide counseling and psychosocial support in home and community settings.
• Help clients navigate real‑life transitions such as returning to work, returning to driving, and increased community participation.
• Support emotional, cognitive, and behavioral adjustment following injury or illness.
• Collaborate with an interdisciplinary team including OT, PT, and SLP.

Responsibilities

• Performs comprehensive evaluations, assessing the social, emotional and support needs of patients and their families.
• Sets measurable objectives that are formulated in conjunction with the rehab team.
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems.
• Instructs/consels patients and families in treating and coping with social and emotional responses associated with illnesses.
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs.
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives.

Qualifications

• Master's Degree in Clinical Social Work (LCSW).
• Current Licensed Clinical Social Worker license in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis.
• CPR certification required.
• Ability to lift 50 pounds.
• Moving, lifting, or transferring of patients which may involve lifting up to 100 pounds following safety procedures.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times.

Salary Range
