Rehab Without Walls Neuro Rehabilitation is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community Program in Seattle. This is a PRN W2 position, offering flexibility with the backing of an interdisciplinary care team.

We provide outpatient‑level services in home and community settings to support recovery and reintegration where people live and thrive. You will work with an expert team to deliver compassionate, evidence‑based care.

Qualifications

• Master's Degree in Clinical Social Work (LCSW).
• Current Licensed Clinical Social Worker licensure in the state of practice.
• Minimum of one year experience in rehabilitation settings serving individuals with traumatic brain injury, spinal cord injury or acute diagnosis.
• CPR certification required.
• Ability to lift 50 pounds.
• Moving, lifting, or transferring patients up to 100 pounds with safety procedures.

Responsibilities

• Provide counseling and psychosocial support in home and community settings.
• Assist clients with transitions such as returning to work, driving, and increased community participation.
• Support emotional, cognitive, and behavioral adjustment after injury or illness.
• Collaborate with an interdisciplinary team including OTs, PTs, and SLPs.

Rehab Without Walls® is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community Program in Seattle. This is a W2 PRN position (not 1099), offering flexibility with the backing and support of an established interdisciplinary care team.

What You’ll Do:

Responsibilities

What you will do: (responsibilities listed are included but not limited to)

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
• Instructs/counsels’ patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient/ and or family to utilize appropriate community resources to achieve measurable objectives

Qualifications

• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• CPR certification required
• Ability to lift 50 pounds
• Moving,lifting,or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills ,visual acuity, and walking/ standing for extended periods
• Additional physical requirements include:pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
