Rehab Without Walls | Home & Community Program is seeking an experienced Licensed Clinical Social Worker (LCSW) for the Bellingham/Seattle area. This PRN W2 role offers flexible scheduling and support from an interdisciplinary care team; care is provided in home and community settings.

The ideal candidate has an MSW with LCSW licensure and rehab experience, and can perform evaluations, counseling, and care coordination with a focus on reintegration and independence.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Current LCSW licensure in the state of practice.
• Minimum one year rehab experience with brain or spinal injuries or acute diagnosis.

Responsibilities

• Provide counseling and psychosocial support in home and community settings.
• Help clients navigate transitions such as returning to work or driving.
• Support emotional, cognitive and behavioral adjustment after injury or illness.
• Collaborate with an interdisciplinary rehab team (OTs, PTs, SLPs).
• Perform comprehensive evaluations of patients and families.
• Identify and connect patients with community resources to meet objectives.

Bellingham, WA

Rehab Without Walls is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community Program in Seattle. This is a W2 PRN position (not 1099), offering flexibility with the backing and support of an established interdisciplinary care team.

Our innovative model brings outpatient level services directly to clients, supporting recovery and reintegration in the environments where life actually happens.

What You'll Do:

• Provide counseling and psychosocial support in home and community settings
• Help clients navigate reallife transitions such as returning to work, returning to driving, and increased community participation
• Support emotional, cognitive, and behavioral adjustment following injury or illness
• Collaborate with an interdisciplinary team including OTs, PTs, and SLPs

Responsibilities:

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
• Instructs/counsels' patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient/ and or family to utilize appropriate community resources to achieve measurable objectives

Qualifications:

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• CPR certification required
• Ability to lift 50 pounds
• Moving,lifting,or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills,visual acuity, and walking/ standing for extended periods
• Additional physical requirements include:pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
