Elara Caring is hiring a Licensed Vocational/Practical Nurse to deliver compassionate, high-quality home health care. You will follow individualized care plans, monitor patient progress, and collaborate with the care team to ensure safe, coordinated care.

This role requires state licensure, a nursing credential, and about one year of clinical experience. Travel between patient homes is expected, with benefits including comprehensive health plans, a 401(k) plan, tuition reimbursement, paid time

Qualifications

• RN/LPN/LVN required by state license.
• Graduate of an accredited nursing program (certificate, diploma, or associate degree).
• 1 year of clinical care experience preferred.

Responsabilités

Job Description

As a Licensed Vocational / Practical Nurse you’ll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
