Elara Caring is hiring a Licensed Vocational/Practical Nurse to join our home health team in Texas. You will follow patient care plans, document care, and coordinate with physicians and families to ensure safe, effective, and compassionate care in patients' homes.

The role requires LVN licensure, at least 1 year of clinical care experience, and the ability to lift up to 50 pounds. Hospice/home health experience is preferred, with driving to patient locations a regular part of work.

• Current State License as a Licensed Vocational/Practical Nurse (LVN)
• Graduate of an accredited Certificate/Diploma/Associate nursing program
• Minimum 1 year of clinical care experience
• Ability to frequently lift, push, pull, and move up to 50 lbs
• Valid driver's license or reliable transportation to travel between patient homes
• Hospice or home health experience preferred

Responsabilidades

• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
