Job summary

Sweetwater Care in Williams, CA is seeking an LVN/LPN to provide nursing care within the scope of practice, following physician orders and center policies.

The role requires Long Term Care experience, PCC charting, and current LVN/LPN license in California with BLS/CPR certification. Residency in Williams is preferred. Experience in nursing management is a plus.

Qualifications

Responsibilities

• Follow established charting procedures and incident reporting.
• Adhere to company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient condition including nutrition, infections, skin conditions, weight loss or other noticeable changes.
• Utilize the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient’s room changes, LOA, new admits, or discharges.
• Make rounds with physicians when necessary.

QUALIFICATIONS

• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.
