Job summary

Bastrop Lost Pines Nursing & Rehabilitation Center is seeking a licensed vocational nurse (LVN) to join the charge nurse team in a skilled nursing setting in Bastrop, TX. The LVN will round with physicians, assess residents, and ensure care plans are implemented while managing nursing staff and incident reporting.

The ideal candidate has an active Texas LVN license, completion of an accredited LVN program, and experience in a skilled nursing facility.

Qualifications

• Completed LVN program at an accredited vocational school or college.
• Active Licensed Vocational Nurse license in the state of Texas in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Administer medications as prescribed.
• Assure implementation of care plans as directed.
• Perform rounds with physician and record visits in clinical records.
• Conduct narcotic reconciliation with incoming and outgoing shifts.
• Oversee aides.
• Initiate and complete Incident and Accident Reports per facility policy.
• Discharge residents per facility policy.
• Communicate with resident’s point of contact for status updates or changes in condition.
• Complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition.
• Assist with other projects as assigned by the Director of Nurses and/or the Administrator.
