Job summary

Klarus Home Health is seeking a PRN/LVN to deliver compassionate nursing care in patients' homes around the Dallas, TX region. You will work under a registered nurse to support diverse patient ages and conditions.

Responsibilities include care coordination, medication administration, and education for patients and families, with an emphasis on accurate documentation and timely communication with the care team.

Qualifications

• Graduate of an accredited School of Vocational/Practical Nursing.
• Current State license as a Licensed Vocational/Practical Nurse or multi-state via NLC.
• Two years' LVN/LPN clinical care experience; home health preferred.
• Proficiency in nursing skills and documentation; reliable transportation.

Description: Klarus Home Health is looking for PRN / Per Diem Licensed Vocational Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in and around the Dallas, TX region and surrounding areas. The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.

Requirements

• Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
• Coordinate care with the interdisciplinary group, patient/family, and referring agency.
• Assist in the development and implementation of patient care plans.
• Monitor patient health and report any changes to the supervising RN.

Job Qualifications

• Graduate of an accredited School of Vocational/Practical Nursing.
• Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.
• Two years' experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.
• HCHB experience highly preferred
• Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
• Reliable transportation and valid, current auto liability insurance.
• Work in patients' homes in various conditions.
• Ability to work a flexible schedule and travel locally.
• Ability to lift up to 50 lbs and move patients.
• Requires working under some stressful conditions to meet deadlines and patient needs.
• Ability to make quick decisions and acquire necessary resources.
• Must meet patient/family individualized psychosocial needs.
