Job summary

Klarus Home Health in Dallas, TX is seeking PRN/LVN/LPN to join our community-based care team delivering nursing services in patients' homes.

Under RN supervision you will coordinate with families and interdisciplinary staff, administer medications, educate families, and document patient progress. This role requires flexibility, local travel, and the ability to lift up to 50 lbs. Home health experience and HCHB familiarity are preferred.

Qualifications

• Graduate of an accredited School of Vocational/Practical Nursing.
• Current State license as an LVN/LPN, or multi-state NLC license when practicing in NLC states.
• Two years’ clinical LPN/LVN experience; home health experience preferred.
• HCHB experience preferred.
• Reliable transportation with valid auto liability insurance.
• Ability to lift up to 50 lbs and travel locally.
• Flexible schedule with local travel.

Responsibilities

• Provide nursing care to patients in their homes under RN supervision.
• Coordinate care with the interdisciplinary group, patient/family, and referring agency.
• Assist in developing and implementing patient care plans.
• Monitor patient health and report changes to the supervising RN.

Klarus Home Health is looking for PRN / Per Diem Licensed Vocational Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in and around the Dallas, TX region and surrounding areas.

The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.

Requirements

Job Qualifications

• Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.
• HCHB experience highly preferred
• Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
• Reliable transportation and valid, current auto liability insurance.
• Work in patients' homes in various conditions.
• Ability to work a flexible schedule and travel locally.
• Ability to lift up to 50 lbs and move patients.
• Requires working under some stressful conditions to meet deadlines and patient needs.
• Ability to make quick decisions and acquire necessary resources.
• Must meet patient/family individualized psychosocial needs.
