Klarus Home Health is seeking licensed practical nurses to provide community-based care to patients in Englewood, Aurora, Lakewood and the Denver metro area. Roles include full-time, part-time, or PRN positions, delivering in-home nursing under RN supervision.

Requirements include an LPN/LVN license, two years of clinical experience (home health preferred), and reliable transportation. EMR experience (HCHB) is highly preferred. Join our dedicated team delivering compassionate home health care.

Formación

• Graduate of an accredited School of Vocational/Practical Nursing.
• Current State license as a Licensed Practical Nurse (LPN/LVN); multi-state NLC preferred if applicable; valid driver’s license.
• Two years’ experience as an LVN/LPN in a clinical setting; home health experience preferred.
• EMR system experience; HCHB preferred.
• Proficient nursing skills and strong interpersonal abilities.

Klarus Home Health is looking for a full time / part time or PRN Licensed Practical Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the Englewood, Aurora, Lakewood and/or Denver metro area.

The primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.

Requirements

Key Responsibilities:

• Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.


• Coordinate care with the interdisciplinary group, patient/family, and referring agency.


• Assist in the development and implementation of patient care plans.


• Monitor patient health and report any changes to the supervising RN.


• Administer medications and treatments as prescribed.


• Educate patients and their families on health care needs, conditions, and options.


• Maintain accurate and timely documentation of patient care and progress.

Job Qualifications:

• Education: Graduate of an accredited School of Vocational/Practical Nursing.


• Licensure: Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.


• Experience: Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.


• EMR system: HCHB experience highly preferred


• Skills: Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.


• Transportation: Reliable transportation and valid, current auto liability insurance.

Environmental and Working Conditions:

• Work in patients' homes in various conditions.


• Ability to work a flexible schedule and travel locally.


• Ability to lift up to 50 lbs and move patients.
