Klarus Home Health & Hospice/Jet Health Inc. is seeking a PRN Licensed Vocational Nurse (LVN/LPN) for weekend coverage in the San Antonio, TX area. The LVN will deliver in-home nursing care under RN supervision, coordinate with families and referral sources, and support ongoing treatment plans.

Qualifications include a valid LVN license, two years of LVN experience (home health preferred), and BLS/CPR as applicable. Travel to patients' homes is required; HCHB EMR experience is preferred.

Qualifications

• Graduate of an accredited School of Vocational/Practical Nursing.
• Current State license as an LVN; multi-state license via NLC if applicable; must have a valid driver's license.
• Two years LVN experience in a clinical care setting; home health experience preferred.
• HCHB EMR experience preferred.
• Proficient nursing skills and good interpersonal skills.

Responsibilities

Klarus Home Health & Hospice/Jet Health Inc is looking for a PRN Licensed Vocational Nurse (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the San Antonio, TX region and surrounding areas. This is a weekend position, Sat/Sun.

The primary function of the LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.

Requirements

Key Responsibilities:

• Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
• Coordinate care with the interdisciplinary group, patient/family, and referring agency.
• Assist in the development and implementation of patient care plans.
• Monitor patient health and report any changes to the supervising RN.
• Administer medications and treatments as prescribed.
• Educate patients and their families on health care needs, conditions, and options.

Job Qualifications:

• Education: Graduate of an accredited School of Vocational/Practical Nursing.
• Licensure: Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.
• Experience: Two years' experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.
• EMR system: HCHB experience highly preferred
• Skills: Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
• Transportation: Reliable transportation and valid, current auto liability insurance.

Environmental and Working Conditions:

• Work in patients' homes in various conditions.
• Possible exposure to blood, bodily fluids, and infectious diseases.
• Ability to work a flexible schedule and travel locally.
• Some exposure to unpleasant weather conditions.

Physical and Mental Effort:

• Prolonged standing and walking required.
• Ability to lift up to 50 lbs and move patients.
• Requires working under some stressful conditions to meet deadlines and patient needs.
• Ability to make quick decisions and acquire necessary resources.
• Must meet patient/family individualized psychosocial needs.
• Requires hand-eye coordination and manual dexterity.
