Job summary

Cantex Continuing Care Network is seeking an LVN/LPN to join the Skilled Nursing team at Mill Creek in Silsbee, TX. You will participate in and oversee patient care within the scope of the LVN/LPN licensure, ensuring safety and quality in daily nursing activities.

Responsibilities

• Participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observe, assess, and report patient conditions/changes and document according to policies and regulations.
• EMR documentation for patient assessment and changes of condition.
• Ensure implementation of care plans and administer medications and treatments as prescribed.
• Supervise CNA staff to ensure appropriate clinical care and patient well-being.

Qualifications: Responsibilities:

• As an LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse, you will participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observes, assesses, and reports Patient conditions/changes and documents in accordance with Cantex’s Clinical Policies & Procedures and state/federal guidelines & regulations.

• Ensures implementation of care plans as directed and administers medications and treatments as prescribed.
• Participates in assigning duties and supervises the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.

Requirements:

• Graduation from an accredited Nursing School
• Valid LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse License
