Job summary

Paradigm Healthcare, a Skilled Nursing Facility, is seeking a dedicated LVN to join our team. Provide direct nursing care under supervision, following the plan of care and physician orders. The LVN will also support daily operations and ensure high-quality resident care.

The ideal candidate holds a Texas LVN license, has LTC experience, and demonstrates leadership and strong clinical skills. Proficiency with Point Click Care and computer literacy are preferred.

Qualifications

• Must hold an active Texas LVN license.
• 2 years in long-term care preferred.
• 6 months in rehabilitative/restorative nursing preferred.
• Demonstrate leadership, organization, and a positive professional attitude.

Responsibilities

• Provide direct nursing care under supervision of a RN or physician.
• Deliver care per resident care plans and physician orders.
• Follow facility policies and nursing service procedures.
• Maintain resident privacy and report PHI violations promptly.
• Perform frequent rounds to ensure nursing staff meet standards.
• Oversee nurse aides and coordinate with staff to uphold policies.
