Job summary

Paradigm at Northwest Nursing Facility in Houston, TX, seeks a dedicated Licensed Vocational Nurse (LVN) to join our day-to-evening shift coverage. You will provide direct care under supervision, follow care plans, and uphold facility policies while ensuring resident safety and dignity.

You’ll lead nursing staff on your unit, communicate effectively with the care team, and support restorative and rehabilitative nursing practices. A supportive workplace with a $3200 bonus for full-time hires.

Qualifications

• Current, unencumbered LVN license in the state.
• 2 years of long-term care experience preferred.
• 6 months rehabilitative/restorative care preferred.
• Ability to lead and maintain a professional attitude.
• Computer literacy and PCC familiarity a plus.

Responsibilities

• Provide direct nursing care under supervision of a RN or physician, per nurse practice act and policies.
• Deliver care to residents following their plan of care and physician orders.
• Follow facility policies and participate in rounds and supervision of staff.
• Protect resident privacy and report PHI violations to the Administrator.
• Oversee nurse aides and coordinate with staff to uphold nursing standards.
• Ensure policies and systems are correctly applied in the department.
