Job summary

Paradigm Healthcare in Houston, TX, seeks a dedicated Licensed Vocational Nurse (LVN) for per diem coverage. You will provide direct nursing care under RN or physician supervision, following residents' plans of care and facility policies.

Ideal candidates hold a current LVN license, have 2 years of long-term care experience, 6 months rehabilitative nursing exposure, demonstrate leadership, and are familiar with Point Click Care and basic computer literacy.

Qualifications

• Must possess a current, unencumbered LVN license in the state.
• 2 years’ Long-Term Care experience preferred.
• Six months of rehabilitative/restorative nursing experience preferred.
• Must demonstrate leadership, organization, and a positive attitude.
• Strong knowledge of Point Click Care preferred.
• Computer literacy: Outlook, Office, Internet.

Responsibilities

• Follow facility policies and nursing service procedures.
• Protect patient privacy and report any PHI violations.

Paradigm Healthcare is a Skilled Nursing Facility that is seeking a dedicated and compassionate LVN to join our team on a PRN basis!

Responsibilities

• Provide direct nursing care, under the direct supervision of a registered nurse or physician, in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgment.
• Deliver care to residents following their plan of care in accordance with current physician's orders, rules, regulations, and guidelines that govern the nursing care facilities.
• Have knowledge of and utilize the facility's written policies and procedures that govern the day-to-day functions of the nursing service department.
• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Make frequent rounds of the assigned unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
• Oversee nurse aides and communication with and oversight of nursing home staff to ensure that specific policies, procedures, and systems are maintained correctly as it relates to the department.

QualificationsRequirements

• Must possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse in the State
• 2 years’ experience in Long-Term Care experience preferred.
• Six (6) months of experience in rehabilitative and restorative nursing practices preferred.
• Must be able to demonstrate leadership, organizational skills, and maintain a positive and professional attitude
• Strong utilization knowledge of Point Click Care preferred
• Computer literacy skills (Microsoft Outlook, Microsoft Office, Internet)
