Job summary

Paradigm Healthcare, a skilled nursing facility, is seeking a dedicated LVN to join our team to provide direct nursing care under the supervision of a registered nurse or physician. You will deliver care to residents per their plan of care, adhere to all policies, and protect patient information while coordinating with staff.

We value leadership, organizational skills, and a positive attitude, with preference for LTC and rehab experience.

Qualifications

• Active LVN license in Texas (unencumbered).
• 2 years’ Long-Term Care experience preferred.
• 6 months’ rehabilitation/restorative nursing experience preferred.
• Demonstrates leadership and organizational skills.
• Maintains a positive, professional attitude.

Responsibilities

• Provide direct nursing care under supervision of RN or physician according to policies and plan of care.
• Deliver care to residents following physician orders and regulatory guidelines.
• Know and follow facility policies and procedures for the nursing service department.
• Protect resident health information and report any violations.
• Make rounds to ensure nursing staff perform to standards.
• Oversee nurse aides and communicate with staff to maintain policy compliance.
• Ensure policies and systems are followed in the department.
