Job summary

Paradigm at the Brazos in Richmond, TX is seeking a dedicated Licensed Vocational Nurse (LVN) to join our skilled nursing team. You will provide direct nursing care under the supervision of a registered nurse or physician, following the resident's plan of care and applicable laws and facility policies.

You should have a valid LVN license in Texas, with 2 years of long-term care experience preferred and familiarity with rehabilitative nursing practices.

Qualifications

• Must possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse in the State of Texas.
• 2 years’ experience in Long-Term Care preferred.
• Six months of experience in rehabilitative and restorative nursing practices preferred.
• Ability to demonstrate leadership, organizational skills, and maintain a positive and professional attitude.
• Strong familiarity with Point Click Care preferred.
• Computer literacy including Microsoft Outlook and Office applications.

Responsibilities

• Provide direct nursing care under the supervision of a registered nurse or physician, following applicable laws, facility policies, and nursing judgment.
• Deliver care to residents according to their plan of care and physicians’ orders, with adherence to rules and guidelines governing nursing care facilities.
• Know and follow the facility’s written policies and procedures for daily nursing operations.
• Protect resident privacy and report suspected or known violations of confidentiality to the Administrator.
• Make frequent rounds to ensure nursing staff perform their duties to acceptable standards.
• Oversee nurse aides and coordinate with staff to maintain proper departmental policies and systems.

Responsibilities

• Provide direct nursing care, under the direct supervision of a registered nurse or physician, in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgment.
• Deliver care to residents following their plan of care in accordance with current physician’s orders, rules, regulations, and guidelines that govern the nursing care facilities.
• Have knowledge of and utilize the facility’s written policies and procedures that govern the day-to-day functions of the nursing service department.
• Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Make frequent rounds of the assigned unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
• Oversee nurse aides and communication with and oversight of nursing home staff to ensure that specific policies, procedures, and systems are maintained correctly as it relates to the department.

QualificationsRequirements
