Job summary

Paradigm Healthcare in Katy, TX is seeking a dedicated Licensed Vocational Nurse (LVN) to join our night shift team at Paradigm at Katy. This full-time position focuses on delivering patient-centered nursing care under the supervision of a RN or physician, following the plan of care and facility policies.

The ideal candidate holds a Texas LVN license, has 2 years in long-term care, and shows leadership and strong communication.

Qualifications

• Active Texas LVN license.
• 2 years’ Long-Term Care experience preferred.
• Six months rehabilitative nursing experience preferred.
• Demonstrates leadership and professional attitude.
• Strong familiarity with Point Click Care.
• Computer literacy: Outlook, Office, Internet.

Responsibilities

• Provide direct nursing care under RN/physician supervision.
• Deliver care per residents' plan of care and physician orders.
• Follow facility policies and procedures.
• Protect resident privacy and report HIPAA violations.
• Make rounds to ensure staff perform to standard.
• Oversee aides and coordinate with staff to maintain department operations.

Responsibilities

• Provide direct nursing care, under the direct supervision of a registered nurse or physician, in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgment.
• Deliver care to residents following their plan of care in accordance with current physician’s orders, rules, regulations, and guidelines that govern the nursing care facilities.
• Have knowledge of and utilize the facility’s written policies and procedures that govern the day-to-day functions of the nursing service department.
• Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Make frequent rounds of the assigned unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
• Oversee nurse aides and communication with and oversight of nursing home staff to ensure that specific policies, procedures, and systems are maintained correctly as it relates to the department.

Qualifications/Requirements

• Must possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse in the State
• 2 years’ experience in Long-Term Care experience preferred.
• Six (6) months of experience in rehabilitative and restorative nursing practices preferred.
• Must be able to demonstrate leadership, organizational skills, and maintain a positive and professional attitude
• Strong utilization knowledge of Point Click Care preferred
• Computer literacy skills (Microsoft Outlook, Microsoft Office, Internet)
