Job summary

Bridge Home Health in Murrieta, CA is seeking a Licensed Vocational Nurse for hospice care on Saturdays and Sundays from 12a to 8:30a. The LVN will provide direct patient care under RN supervision, implement the plan of care, and document accurately.

Requirements include graduation from an accredited LPN/LVN program, 3 years nursing experience (home health preferred), active California license, CPR, and a valid driver's license.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience; home health preferred.
• Currently licensed as LPN/LVN in California.
• CPR certification is maintained and valid.

Responsibilities

• Provide direct patient care under RN supervision following the care plan.
• Document accurately and in a timely manner per the plan of care.
• Educate patients and families on self-care and diagnoses as appropriate.
• Coordinate with the team to identify needs and report to the RN/Clinical Manager.
• Utilize equipment and supplies with aseptic technique and efficiency.
• Perform other duties as assigned by the RN.

Job Description Summary

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Essential Job Functions/Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Manager.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

Position Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Possess and maintain valid CPR certification.
• Complies with accepted professional standards and practice.
• Demonstrates good verbal and written communication, and organization skills.
• Must have a valid California Driver's License with automobile that is in good working order and insured in accordance with the organization requirements.
