Job summary

Bridge Home Health is seeking a Licensed Practical Nurse (LPN) to provide direct patient care under supervision of a registered nurse in home health settings in California. The role requires following the plan of care, administering treatments, and collaborating with the care team to support positive patient outcomes.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
• Currently licensed as an LPN or LVN in California.
• Possess and maintain CPR certification.
• Demonstrates good verbal and written communication, and organization skills.
• Must possess a valid California Driver’s License with automobile in good working order and insured.

Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family education regarding self-care and diagnosis and plan of care.

• Uses and maintains equipment adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the RN.

Job Description Summary

Essential Job Functions/Responsibilities:

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Manager.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.
