Job summary

Adaptive Aids, LLC is seeking a Licensed Vocational Nurse (LVN) to provide direct nursing care within our Home and Community-based Services (HCS) program in Houston. You will work under RN supervision, administering medications and coordinating services to support individualized care plans.

The role emphasizes patient safety, meticulous documentation, HIPAA compliance, and collaboration with physicians, families, and the care team.

Qualifications

• Active Texas LVN license in good standing.
• Current CPR/BLS certification.
• Minimum 1 year of nursing experience preferred.
• HCS, IDD, or home health experience preferred.
• Valid Texas Driver’s License or State ID.
• Ability to pass background and Nurse Aide Misconduct Registry checks.
• Reliable transportation.
• Strong communication and organizational skills.
• Ability to lift, transfer, and reposition clients as needed.
• Ability to stand, walk, bend, and perform physical caregiving tasks for extended periods.
• Ability to respond to emergency situations quickly and appropriately.

Responsibilities

• Provide nursing care that promotes client safety and comfort.
• Administer medications and treatments (e.g., wound care, catheter care, vitals monitoring).
• Monitor and report changes in client condition to RN/care team.
• Assist in developing and implementing individualized care plans.
• Coordinate services such as therapy appointments and follow-ups.
• Communicate with physicians, families, and interdisciplinary teams.
• Support direct care staff in emergencies and proper care procedures.
• Complete accurate and timely documentation in electronic systems.
• Ensure HIPAA-compliant documentation and regulatory compliance.
