Qualifications

• Current Texas LVN license (unrestricted).

• Willingness to travel in assigned territory daily.

Responsibilities

• Deliver skilled nursing visits in the home within LVN scope under RN supervision.
• Perform focused assessments and monitor condition changes.
• Escalate promptly to RN/MD with clear, objective information.
• Support medication reconciliation and safety education.
• Provide patient and caregiver education aligned to the care plan.
• Coordinate care with office and interdisciplinary team; document timely.
