Job summary

Adaptive Aids, LLC is seeking a Licensed Vocational Nurse (LVN) to provide direct nursing care in our Home and Community-based Services (HCS) program. This role involves medication administration, documentation, and coordination of services to support individualized care plans under RN supervision.

The LVN will engage in client care across home health settings, ensuring compliance with HCS and HHSC standards, while maintaining professionalism and patient safety at all times.

Qualifications

• Provide direct nursing care to clients in Home and Community-based Services.
• Administer medications and treatments per care plan.
• Complete timely and accurate nursing documentation.
• Coordinate services with physicians, families, and care teams.

Responsibilities

• Deliver client-centered nursing care in home/community settings.
• Monitor client conditions and report changes to RN and care team.
• Maintain infection control and compliance with HCS regulations.
