AltaMed Health Services Corp. in Los Angeles, CA is seeking a Licensed Vocational Nurse, Home Care Coordinator to develop and implement homecare services for program participants, coordinating contracted home services, DME, and nursing home care.

You will attend IDT and coordination meetings to ensure seamless patient care. The ideal candidate has LVN licensure in California, at least one year of LVN experience, and a valid driver’s license with a good record; BLS certification is required or

• Minimum one year of LVN nursing experience.
• Experience with physical, mental, and social needs of patients.
• At least one year serving frail or elderly populations.
• Valid California driver’s license with good driving record.
• BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

Job Overview

Responsible for the development and implementation of homecare services for program participants, including the coordination of all contracted home services, durable medical equipment, and nursing home care. Attending scheduled IDT and Coordination meetings.

Minimum Requirements

• 
  

  Graduation from an accredited LVN school, coupled with a current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians required.

  A minimum of one (1) year of experience practicing as a Licensed Vocational Nurse.

  
  


• 
  

  Experience and knowledge regarding the physical, mental, and social needs of the patients are highly desirable.

  At least one year of working with a frail or elderly population

  Valid California driver's license with a good driving record.

Compensation

Pay for this job starts at $31.93 hourly
