Job summary

AltaMed is seeking an LVN to develop and implement homecare services for program participants in California. You will coordinate contracted home services, durable medical equipment, and nursing home care, ensuring patient needs are met.

The role requires an active LVN license in California and at least one year of LVN experience, with a valid CA driver’s license and a current BLS certification. IDT and coordination meetings are part of the responsibilities.

Qualifications

• Graduation from an accredited LVN program with CA LVN license.
• At least 1 year of licensed LVN experience.
• Experience with frail or elderly populations preferred.
• Valid California driver's license with clean driving record.
• Current BLS certification or higher.

Responsibilities

Job Overview

Responsible for the development and implementation of homecare services for program participants, including the coordination of all contracted home services, durable medical equipment, and nursing home care. Attending scheduled IDT and Coordination meetings.

Minimum Requirements

• Graduation from an accredited LVN school, coupled with a current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians required.
• A minimum of one (1) year of experience practicing as a Licensed Vocational Nurse.
• Experience and knowledge regarding the physical, mental, and social needs of the patients are highly desirable.
• At least one year of working with a frail or elderly population
• Valid California driver’s license with a good driving record.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

Pay for this job starts at $31.93 hourly
