Job Title: LVN Charge Nurse

Location: Cityview Nursing & Rehabilitation Center – Fort Worth, TX 76132

Position Type: PRN

Primary Responsibilities

Observes, assesses, documents, and reports resident condition/changes during rounds.
Assures implementation of care plans as directed.
Administers medications as prescribed.
Conducts rounds with physician and records visits in clinical records.
Performs narcotic reconciliation with incoming and outgoing shifts.
Identifies significant changes in resident condition and takes necessary action per facility policies and procedures.
Oversees and monitors aides.
Initiates and completes Incident and Accident Reports per facility policy.
Discharges residents per facility policy.
Completes Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition.
Communicates with resident’s point of contact when requested for a status update or when there is a change in condition.
Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator.
