Job summary

Alsana is hiring a Licensed Vocational Nurse for an evening shift in Thousand Oaks, CA. You will provide direct nursing care, administer medications, perform phlebotomy, and document all assessments in our EMR.

You will work with a multidisciplinary team to support adults and adolescents in recovery, monitor nutrition and safety, and coordinate with pharmacy for medications and tests.

Qualifications

• IV certification within 30 days of start date.
• Valid license as a Vocational Nurse.
• 1+ years hospital-based clinical experience preferred.
• Current CPR Certification preferably through the American Heart Association.
• 1+ years clinical experience in a mental health or residential facility is preferred.

Responsibilities

• Provide nursing care including medication administration and phlebotomy.
• Document nursing assessments in the EMR.
• Work with a multi-disciplinary team to support clients.
• Conduct daily physical and mental status assessments and document them.
• Complete health questionnaires and record in the medical record.
• Administer medications, monitor for side effects and responses.
• Record weights and vital signs in EMR; verify unusual readings.
• Coordinate with pharmacy for med deliveries and questions.
• Monitor nutrition and dietary needs; report non-compliance.
• Organize consultations with physicians for tests and provide diagnoses for labs.
• Complete nursing assessments, care plans, SOAP notes; ensure co-signature by Nurse Manager/RN.
• Administer first aid per policy and follow up documentation.
• Obtain insurance updates for precertification.
• Maintain communication with clients, families and staff; hand-offs between shifts.
• Perform other duties as assigned.

What You’ll Do

As the Licensed Vocational Nurse at Alsana, you will provide direct nursing care that includes and is not limited to medication administration, phlebotomy, and documenting all nursing related assessments in the EMR. You will be part of a multi‑disciplinary team to support the needs of Alsana clients.

• Conduct daily physical and mental status assessments and document them accordingly.
• Complete health questionnaires and accurately document in the medical record.
• Administer medication, observe for side effects and client response, documenting observations in the medical record.
• Create and maintain weight and vital signs in EMR and check daily for accuracy. Recheck any documented weight or vital sign that is unusual and document.
• Participate in client observations to promote abstinence of behaviors and safety.
• Coordinate with pharmacy for medication deliveries and questions regarding medications. Fax additional information as needed for payments according to the HIPAA protocols and facility policy.
• Monitor the nutritional and dietary needs of the clients and report non‑compliance with the plan of care.
• Organize, coordinate, and follow up on all consultations with physicians/medical personnel for tests, etc., and provide the appropriate diagnosis for laboratory, radiology, and other diagnostic testing.
• Complete all Alsana Nursing Assessments, Care‑plans, S.O.A.P Notes, Admission Note, Discharge Notes, Noc Shift note and accurately document in the medical record. All LVN Notes to be co‑signed by the Nurse Manager and/or RN.
• Administer first aid according to facility policy, including all appropriate follow‑up documentation.
• Obtain insurance updates to get precertification for medications and other coverage.
• Establish and maintain good communication with clients, families, and staff. Participate in hand‑off communication from shift to shift.
• Perform all other duties as assigned.
