Job summary

Fairview Health Services in Minneapolis, Minnesota is seeking a Licensed Programmatic Care Therapist to provide clinical interventions, group therapy, and education for patients and families in PHP and IOP settings at Riverside location. The role emphasizes treatment planning and transition/discharge planning within an interdisciplinary team.

Applicants must hold a Master's in Social Work or Psychology, have at least two years of experience, and be BLS certified before starting.

Qualifications

• Masters in Social Work and/or Psychology required.
• Two years’ experience required.
• BLS certification required prior to starting.
• Minnesota license: LICSW, Psychologist, LPCC, or LMFT required.
• De-escalation and self-defense training for outpatient units.
• Ability to supervise.

Responsibilities

• Determine clinical interventions and facilitate group therapy.
• Develop transition/discharge plan and manage each case.
• Interdisciplinary team member in treatment planning.
• Educate patients and families about care planning and psycho-education.
