Job summary

MIND 24-7 in Mesa, AZ seeks a Licensed Therapist to deliver direct clinical care through individual and group therapy, case management, and treatment planning. You will collaborate with a multidisciplinary team to ensure holistic, patient-centered care and timely documentation in a fast-paced setting.

You will provide psychosocial assessments, discharge planning, and coordination with nurses and BHMPs, while educating families to maintain continuity of care.

Qualifications

• Master’s degree in Social Work, Counseling, or related behavioral health field.
• Current licensure in Arizona (LCSW, LMSW, LPC, LAC, LMFT).
• Strong clinical assessment, planning and documentation skills.
• Knowledge of Medicaid, Medicare, and regional behavioral health systems.
• Experience with crisis stabilization, case management, and therapeutic interventions.
• Ability to work well in fast-paced, high-acuity environments.
• Level 1 Fingerprint Clearance Card (or ability to obtain upon hire).

Responsibilities

• Provide individual and group therapy services to patients in crisis or in need of urgent care.
• Complete psychosocial assessments and develop individualized treatment plans.
• Participate in clinical staffing and discharge planning with care team.
• Coordinate care with nurses, navigators, BHMPs, and other clinical professionals.
• Educate and collaborate with patients’ families to ensure successful continuity of care.
• Identify and address social determinants of health and resource needs.
• Maintain timely and accurate documentation in compliance with regulatory standards.
• Assist with referrals, care coordination, and follow-up support as needed.
