Job summary

PAX Health is seeking a licensed Therapist to deliver evidence-based clinical services in an outpatient setting. You will work with individuals, families, and couples, including Workers’ Compensation or motor vehicle cases as needed.

Responsibilities include intake reviews, biopsychosocial assessments, diagnosis, treatment planning, and coordination with a multidisciplinary team. Licensure (LCSW/LPC) or supervision under a licensed supervisor is required where applicable.

Qualifications

  • Master’s degree in Social Work, Counseling, Psychology, or related field.
  • Active and unrestricted clinical license or provisional license (LCSW, LPC).
  • Fully licensed clinicians must be able to assess, diagnose, and treat patients independently.
  • Strong clinical skills in assessment, treatment planning, and evidence-based modalities.
  • Interest and/or experience in CBT, DBT, ACT, trauma-informed care, or similar approaches.
  • Experience with diverse patient populations and mental health conditions preferred.
  • Eligible to provide services for Workers’ Compensation cases preferred.
  • Strong communication and interpersonal skills.
  • Strong organizational, time management, and documentation skills.
  • Proficiency with EHR systems.
  • Ability to collaborate within a multidisciplinary team.
  • Commitment to ethical practice, supervision, and ongoing learning.
  • LCADC or CADC is a plus.

Responsibilities

  • Receive, review, and respond promptly to assigned intake referrals
  • Conduct biopsychosocial assessments and support diagnostic formulation
  • Establish diagnoses and develop individualized treatment plans appropriate to licensure levels
  • Provide direct clinical services including individual, group, family, and/or couples therapy
  • Maintain an active caseload appropriate to experience level and licensure status
  • Independently manage treatment planning when fully licensed
  • Participate in supervision when provisionally licensed
  • Monitor patient progress and adjust treatment approaches based on outcomes
  • Support discharge and aftercare planning for continuity of care
  • Provide case management and referrals to external resources when appropriate
  • Maintain accurate, timely, compliant documentation in the EHR
  • Ensure coordination of services across providers and disciplines
  • Collaborate with prescribers and multidisciplinary teams to support outcomes
  • Participate in team meetings, consultations, training, and professional development
  • Comply with confidentiality and regulatory requirements
  • Deliver culturally responsive, patient-centered care in line with PAX Health values

The Therapistis responsible for delivering high quality,evidence basedclinical services to patients and their families within PAX Health’s outpatient setting. This includes individual, group, family, and/or couples therapy tailored to each patient’s needs and level of care, as well as Worker’s Compensation clients or Motor VehicleAccidentclients as needed.

This position may be filled by a fully licensed clinician (LCSW, LPC). Fully licensed clinicians are expected to practice independently, managing their own caseloads and clinical decision making, while provisionally licensed clinicians work under the supervision of a licensed Clinical Supervisor as they continue developing clinical competencies toward independent licensure.

The Therapistplays a key role in assessment, diagnosis, treatment planning, ongoing care coordination, and discharge planning. In partnership with clinical leadership and the multidisciplinary treatment team, the Therapist ensures services are delivered in alignment with organizational standards, regulatory requirements, ethical guidelines, andevidence basedpractices, whilemaintaininga patient centered and culturally responsive approach to care.
