PAX Health in New Castle, PA; Freehold, NJ is seeking an outpatient Therapist to deliver evidence-based individual, group, family, and couples therapy in a patient-centered setting. You will conduct biopsychosocial assessments, formulate diagnoses, create tailored treatment plans, and coordinate with a multidisciplinary team.

Licensed or provisionally licensed clinicians are encouraged to apply; independent practice is expected for fully licensed providers.

Qualifications

  • Master’s degree in Social Work, Counseling, Psychology, or related field.
  • Active and unrestricted clinical license or provisional license.
  • Licensed clinicians must practice independently where applicable.
  • Strong skills in assessment, treatment planning, and evidence-based modalities.
  • Experience with CBT, DBT, ACT, trauma-informed care.
  • Experience with diverse patient populations and mental health conditions.
  • Eligible for Workers’ Compensation cases preferred.
  • Strong communication and interpersonal skills.
  • Organizational, time management, and documentation skills.

Responsibilities

  • Receive, review, and respond promptly to intake referrals.
  • Conduct biopsychosocial assessments and support diagnostic formulation.
  • Establish diagnoses and develop individualized treatment plans.
  • Provide direct clinical services: individual, group, family, and couples therapy.
  • Maintain an active caseload appropriate to licensure status.
  • Independently manage treatment planning when fully licensed.
  • Participate in supervision as required for licensure status.
  • Monitor patient progress and adjust treatment approaches.
  • Support discharge and aftercare planning for continuity of care.
  • Coordinate referrals to external resources when appropriate.
  • Maintain accurate, timely documentation in the EHR.
  • Collaborate with prescribers and multidisciplinary teams to improve outcomes.

Education

Master’s degree in Social Work, Counseling, Psychology, or a related field

The Therapistis responsible for delivering high quality,evidence basedclinical services to patients and their families within PAX Health’s outpatient setting. This includes individual, group, family, and/or couples therapy tailored to each patient’s needs and level of care, as well as Workers’ Compensation clients or Motor VehicleAccidentclients as needed.

This position may be filled by a fully licensed clinician (LCSW, LPC). Fully licensed clinicians are expected to practice independently, managing their own caseloads and clinical decision making, while provisionally licensed clinicians work under the supervision of a licensed Clinical Supervisor as they continue developing clinical competencies toward independent licensure.

The Therapistplays a key role in assessment, diagnosis, treatment planning, ongoing care coordination, and discharge planning. In partnership with clinical leadership and the multidisciplinary treatment team, the Therapist ensures services are delivered in alignment with organizational standards, regulatory requirements, ethical guidelines, and evidence based practices, while maintaining a patient centered and culturally responsive approach to care.

Responsibilities:

  • Establish diagnoses and develop individualized treatment plans appropriate to level of licensure and supervision requirements
  • Provide direct clinical services, including individual, group, family, and/or couples therapy
  • Maintain an active caseload appropriate to experience level and licensure status
  • Independently manage treatment planning and clinical decision making when fully licensed
  • Participate in individual and group supervision in accordance with licensure requirements when provisionally licensed
  • Monitor patient progress and adjust treatment approaches based on clinical need and outcomes
  • Support discharge and aftercare planning to ensure continuity of care
  • Provide case management services and coordinate referrals to external resources when appropriate
  • Maintain accurate, timely, and compliant documentation within the Electronic Health Record (EHR)
  • Ensure coordination and integration of services documented across providers and disciplines
  • Collaborate with prescribers, including the Medical Director, and multidisciplinary care teams to support patient outcomes
  • Participate in treatment team meetings, clinical consultations, staff meetings, trainings, and professional development activities
  • Comply with all local, state, and federal confidentiality and regulatory requirements
  • Deliver culturally responsive, patient centered care aligned with PAX Health values
  • Abide by PAX Health code of ethics and model values
  • Perform other duties as needed within the scope of the role

  • Interest and/or experience in modalities such as CBT, DBT, ACT, trauma informed care, or similar approaches
  • Experience working with diverse patient populations and a range of mental health conditions preferred
  • Eligible to provide services for Workers’ Compensation cases preferred
  • Strong communication and interpersonal skills
  • Strong organizational, time management, and documentation skills
  • Proficiency with EHR systems

Locations:
  • New Castle, PA: Compensation is 55% of collections, part-time or full-time
  • Freehold and Point Pleasant NJ: Compensation is $65 per hour, part-time
