Job summary

AppleGate Recovery, a Baymark Health Services company, seeks a compassionate Full Time Licensed Substance Abuse Counselor to support patients in Medication Assisted Treatment for opioid use disorder at our outpatient clinic. You will work with a multidisciplinary team to implement individualized care plans and improve quality of life.

The role emphasizes trauma-informed practices, evidence-based treatments, and collaboration with physicians and nurses to monitor progress and coordinate services.

Qualifications

• Must hold at least one Kentucky license: LCADC, CADC, LCSW/CSW, LMFT/LMFTA, LPCC/LPCA.
• Prior experience with chemical dependency is required.
• Positive, non-judgmental attitude toward patients is essential.

Responsibilities

• Provide individual and group counseling; coordinate services per patient need and state regulations.
• Conduct initial assessment to determine eligibility for services.
• Complete bio-psychosocial assessment and establish an individualized treatment plan.
• Review patient progress and respond to grievances and complaints.
• Develop aftercare and discharge plans; other duties as required by supervisor.
