Position Details: Licensed Speech-Language Pathologist (SLP) - Care Coordination Program

We’re seeking Speech-Language Pathologists to support our innovative care coordination program! This role allows you to practice at the top of your license while engaging in duties that standardize hours and foster career growth. Responsibilities
  • Provide speech-language pathology and wellness services for older adults.
  • Conduct assessments, develop treatment plans, and deliver therapeutic interventions as part of a multidisciplinary team.
  • Support care coordination by conducting wellness checkups, communicating with providers and caregivers, and assisting with telehealth visits.
  • Maintain professional and ethical standards in line with company policies and initiatives.
  • Contribute to program development and quality improvement efforts.
  • Enhance each patient’s functional well-being and satisfaction.
Your Schedule

While our clinics typically operate from 8 a.m. to 5 p.m., Monday through Friday, our flexible scheduling allows you to tailor your work hours to your lifestyle.
