Job description

Speech-Language Pathologist (SLP)
Part-Time | ActivCare Bressi Ranch & ActivCare 4S Ranch

Some of the most important things we do as clinicians have nothing to do with "fixing" someone.

Sometimes it's helping someone find the words.

Sometimes it's helping a family understand how to communicate differently.

Sometimes it's helping a resident stay safe at the dining table.

Sometimes it's finding a way for someone living with dementia to continue participating in the world around them.

That's speech therapy in senior living.

And that's exactly what we're looking for at ActivCare Communities.

Prava is seeking a part-time Speech-Language Pathologist (SLP) to join our clinical team at some of the most thoughtfully designed senior living communities.

ActivCare Communities are built around a simple promise: to love and care for each family as they do their own.

Their model combines highly individualized care, 24/7 nursing, specialized memory care, and the ability to support residents as their needs change over time.

That makes this a remarkable environment for an SLP.

This is not a traditional SLP role.

You won't be treating people in isolation from their everyday lives.

You'll be working in their home.

You'll see how communication, cognition, swallowing, and functional independence actually affect the resident's day.

You'll work alongside caregivers who know residents deeply.

You'll collaborate with nurses, families, activities and life enrichment teams, and other clinicians.

And you'll have the opportunity to make your therapy functional in a way that is difficult to replicate in a traditional outpatient environment.

Your work may look like:
  • Helping a resident with dementia communicate a need that others are struggling to understand

  • Developing strategies that help a resident participate more successfully in conversation

  • Supporting cognitive-communication skills that allow a resident to remain as independent as possible

  • Evaluating and treating swallowing and feeding-related concerns within your scope of practice

  • Helping caregivers understand how to communicate with residents experiencing cognitive change

  • Creating environmental and communication strategies that reduce frustration and increase participation

  • Supporting safe and meaningful mealtimes

  • Helping families understand what is changing—and what can still be preserved

  • Collaborating with the interdisciplinary team when a resident's function or communication changes

  • Helping residents remain connected to the people and activities that matter to them

We want you to think beyond the diagnosis.

A resident may have dementia.

But dementia isn't the goal of therapy.

A resident may have dysphagia.

But dysphagia isn't the goal of therapy.

A resident may have aphasia or cognitive-communication deficits.

But those aren't the reason we're here.

The reason we're here is the person.

Maybe it's being able to tell a story.

Being understood by a spouse.

Participating in a favorite activity.

Sharing a meal safely with friends.

Making a choice.

Expressing a need.

Laughing at a joke.

Remaining part of the community.

Those moments matter.

ActivCare Communities specifically emphasizes individualized care based on each resident's physical, cognitive, social, emotional, and spiritual needs, with a strong focus on dignity and preserving independence.

We want an SLP who believes in that same philosophy.

This is where clinical creativity matters.

ActivCare Communities support residents with a wide range of needs, including individuals living with different stages of memory loss and residents requiring more complex assistance.

That means you won't see the same clinical picture every day.

You'll need to think.

Adapt.

Problem-solve.

Communicate.

And sometimes throw the textbook out and ask:

"What will actually work for this person?"

That's the kind of clinician we're looking for.

Who are you?

You're a California-licensed Speech-Language Pathologist who:

  • Loves working with older adults

  • Has strong clinical skills in cognition and/or swallowing

  • Understands dementia and neurodegenerative conditions—or wants to become exceptionally good at working with them

  • Thinks functionally rather than simply diagnostically

  • Is comfortable collaborating with caregivers and families

  • Can translate complex clinical information into language people actually understand

  • Is creative, patient, and curious

  • Can work independently while remaining highly collaborative

  • Takes pride in the quality of their clinical work

  • Believes dignity and autonomy are part of clinical care—not something separate from it

Senior living experience is a plus.

But we care more about how you think than where you've worked.

Why Prava?

Because we're building a different way to deliver rehabilitation in senior living.

Prava gives clinicians the infrastructure to focus on what matters most:

The resident.

You'll have access to:

Modern technology

A therapy platform designed around clinicians, including AI-enabled documentation tools.

Clinical infrastructure

Standards, resources, mentorship, and a growing clinical community.

Operational support

Prava handles the infrastructure behind the scenes so you can spend more of your time doing clinical work.

An embedded model

You're not a therapist who drops in, treats, and disappears.

You become part of the community.

A chance to grow

Even in a part-time role, you'll have opportunities to grow clinically, contribute to the Prava model, and expand your role as the community and program grow.

The best SLPs don't just treat communication.

They protect connection.

They help people remain understood.

They help families stay connected.

They help residents maintain choice, dignity, and participation—even as their abilities change.

That's meaningful work.

And at ActivCare Communities, there's a lot of meaningful work to do.

The opportunity
