You will work within Cambridge Village’s Optimal Living community, delivering skilled PT aligned with residents’ goals, collaborating with an interdisciplinary team, and leveraging technology to support high-quality care.

Responsibilities

  • Evaluate residents and develop individualized plans of care.
  • Provide skilled physical therapy across a range of functional needs.
  • Treat balance, gait, strength, mobility, endurance, and functional limitations.
  • Help residents recover after illness, injury, hospitalization, or other changes in function.
  • Develop treatment plans around meaningful functional goals.
  • Provide resident and caregiver education.
  • Collaborate closely with community staff and other healthcare professionals.
  • Identify changes in mobility or function that may benefit from intervention.
  • Support fall prevention, mobility, and wellness initiatives.
  • Participate in the community when opportunities arise—not just in the therapy space.
  • Communicate clearly with residents, families, and the interdisciplinary team.
  • Use Prava's technology and AI-enabled documentation tools to reduce administrative burden.

As a Prava Physical Therapist, you’ll become an important clinical presence within the Cambridge Village community.

You’ll build relationships with residents, families, wellness professionals, community leadership, and other clinicians.

Senior living experience is a plus.
