Lions Rehab Center in Cumberland, Maryland is seeking a compassionate Social Worker (LCSW-C) to join our 101-bed skilled nursing facility. You will provide social work services to residents and patients, focusing on their physical, emotional, and social well-being.

The role includes developing care plans, staying current with regulations, and supporting admissions and family education. We value a person-centered approach and a collaborative work environment at our facility.

Qualifications

• Bachelor's degree from an accredited school of Social Work or related field is required.
• Must possess any licenses/certifications as required by the state to practice in long-term care.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct social services programs.
• Develop and coordinate department policies and procedures.
• Maintain up-to-date knowledge of federal and state regulations and professional standards.
• Prepare social histories, assessments, and care plans for residents.
• Assist in admissions processes and maintain waiting lists as needed.
• Provide tours and information to prospective families and residents.
