Job summary

CMR Recruiting is seeking an LCSW-C for a Skilled Nursing Facility in Baltimore, MD. This full-time, on-site role offers competitive pay and comprehensive benefits, including medical, dental, vision, life insurance, disability insurance, a 401K with match, PTO and CEU reimbursement.

The LCSW-C will provide psychosocial assessments, care and discharge planning, and direct support for residents and families, handling a caseload that includes substance use and behavioral health needs.

Qualifications

• Active Maryland LCSW-C license is required (LMSW/LGSW do not meet the requirement).
• Comfort working with residents who have substance use and behavioral health needs.
• Knowledge of psychosocial assessment, care planning and discharge planning.
• Strong documentation and communication skills.
• Skilled nursing or long-term care experience is a plus but not required.

Responsibilities

• Complete psychosocial assessments and the social services sections of the MDS.
• Build and update care plans with the interdisciplinary team.
• Coordinate discharge planning and community resources.
• Support residents and families through admission, stay and transition.
• Maintain documentation and regulatory compliance.
