Cantex Continuing Care Network is seeking a Social Services Worker for Fort Bend HealthCare Center in Rosenberg, TX. The role focuses on identifying medically related social and emotional needs of patients/residents and coordinating discharge planning with care teams.

You will develop social histories, collaborate with physicians, and provide individual and group support to promote well-being and successful transitions from the facility.

Qualifications

• Bachelor's degree in Social Work or related field.
• Licensed per state requirements or eligible for licensure.
• Skills in communication, documentation, assessment, and social work methods and techniques.
• At least one year of experience in long-term care or related healthcare setting.
• Ability to read, write, and speak the English language.

Responsibilities

• Develops social histories and social assessments to identify needs and plan care.
• Participates in interdisciplinary care plan conferences and contributes to plans.
• Establishes social action plans and goals aligned with emotional needs.
• Designs social service programs meeting needs and complies with regulations.
• Interprets and promotes patients' rights with the Administrator.
• Provides individual and group support for patients and families.
