Job summary

Qualifications

• Master's degree in Social Work is required.
• State licensure as LMSW or LCSW upon hire is required.
• CPR certification upon hire is required.

Responsibilities

• Identify community resources and coordinate referrals for patients.
• Ensure timely transition to appropriate levels of care.
