Responsibilities

• Develops social histories and social assessments with care plans.
• Participates in interdisciplinary care plan conferences and contributes to care plans.
• Establishes social action plans and goals aligned with needs.
• Designs social service programs meeting medically related social needs.
• Coordinate discharge planning with staff, patients, and families.
• Complete progress notes, intervention notes, and adjustment notes per regulations.
