Southpark Meadows Nursing and Rehabilitation Center in Austin, TX is seeking a compassionate Licensed Social Worker (LSW) to join our skilled nursing team. You will support residents’ emotional and social needs from admission through discharge as part of the interdisciplinary care team.

The role emphasizes psychosocial assessments, care planning participation, advance care planning, and collaboration with the Business Office Manager on financial and social challenges.

Qualifications

• Bachelor’s or Master’s degree in Social Work.
• Licensed Social Worker (LSW) in Texas.
• Experience in skilled nursing facilities preferred.
• Strong communication, documentation, and problem-solving skills.
• Compassionate demeanor supporting residents through transitions.

Responsibilities

• Build relationships with residents and families to assist with admissions and care planning.
• Complete psychosocial assessments and participate in care plan meetings.
• Assist with advance care planning and end-of-life discussions.
• Facilitate referrals for vision, hearing, dental, or behavioral health services.
• Assist with financial or social challenges with the Business Office Manager.
• Address concerns and collaborate on conflict resolution with the Administrator.
• Conduct suicidal/homicidal ideation assessments and follow up.
• Assist with MDS and QA documentation as needed.
• Round each morning before department meetings to check in with residents.
• Serve as Manager on Duty on rotating weekends.
