Job summary

Blackwell Talent is seeking a Licensed Social Worker for a skilled nursing facility in Austin, Texas. You will assess residents, develop care plans, support families through transitions, and advocate for resident rights.

Collaborate with nursing, therapy, and administration to ensure holistic care, provide counseling and resource navigation, and maintain accurate documentation in accordance with regulatory standards.

Qualifications

• Active LCSW in Texas or eligible for licensure.
• Experience in skilled nursing/long-term care preferred.

We’re seeking a compassionate and motivated Licensed Social Worker to join a skilled nursing team in Austin, TX. This is an opportunity for a Licensed Social Worker who is passionate about supporting residents and families through care transitions, psychosocial support, and advocacy in a fast-paced healthcare environment.

What You’ll Do:

• Provide psychosocial assessments for residents and develop individualized care plans
• Support residents and families through adjustment to illness, long-term care placement, and end-of-life planning
• Coordinate discharge planning and safe transitions of care in collaboration with the interdisciplinary team
• Serve as a key advocate for resident rights, needs, and quality of life
• Provide counseling, emotional support, and resource navigation for residents and families
• Maintain accurate and timely documentation in compliance with regulatory standards
• Collaborate closely with nursing, therapy, and administration to ensure holistic resident care

What We’re Looking For:

• Active Licensed Clinical Social Worker (LCSW) in Texas (or eligible for licensure)
• Experience in skilled nursing, long-term care, hospice, or healthcare setting preferred
• Strong communication, assessment, and crisis intervention skills
• Ability to work effectively in a multidisciplinary team environment
• Passion for advocacy, resident support, and improving quality of life
