In the rich Franciscan tradition of compassionate, quality care, Franciscan Care Center is a residential and rehabilitation facility for the care of the elderly and those in need of extensive rehabilitation services. We are seeking a Licensed Social Worker (LSW) to join our team! This position requires an active LSW (Licensed Social Worker) license in the state of Ohio.

Job Responsibilities

• Help assess resident needs and develop individualized care plans.
• Provide emotional support and guidance to residents and families.
• Coordinate with the interdisciplinary care team to ensure holistic care.
• Assist with admissions, discharges, and transitions of care.
• Maintain accurate, confidential documentation and resident records.
• Communicate with outside agencies to connect residents with resources.
• Participate in care plan meetings and departmental discussions.
• Support the overall well-being of our residents through presence and compassion.
• In addition to the essential job specific duties listed above, the Licensed Social Worker (LSW) shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission and values (compassion, excellence, inclusion, integrity, collaboration) of CHI Living Communities and will be evaluated on such basis. Job Requirements

• Bachelor’s Degree in Social Work from an accredited college or university.
